I, the undersigned do hereby respectfully submit my application to the officers and members of the Pennsylvania Criminal Investigator's Lodge #92, Fraternal Order of Police, and do humbly show that I am desirous of being a member of your honorable lodge and request that I may be received among you , and I will ever pray for the prosperity and glory of the fraternity and the welfare of the brothern. I do certify that I am an American citizen of full age and good moral character and have never been convicted of any felony or misdemeanor or for any offense punishable by imprisonment. I do certify that I am a full time or retired peace officer for the Commonwealth of Pennsylvania If elected to membership, I promise to abide by the by-laws of Lodge #92 and all the rules and regulations of said Lodge, and pay all dues established by the membership, and to conduct myself at all times in such a manner as not to bring reproach upon the Fraternal Order of Police, Lodge #92 or myself.
I also agree that violation of that pledge shall result in forfeiture of my membership and all its privileges. If my membership should be revoked for cause, I do hereby consent to immediately return to the lodge all emblems and cards signifying membership in Lodge #92. I have read the foregoing application and in testimony of my full understanding thereof, have, of my own free will and hand, affixed my signature thereto.
